For every project completed, licensed polyurethane applicators are required to fill out a "Daily Work Record" and document their spray foam installation and submit it to their governing licensure. At Polysource, we are licensed through UFC (Urethane Foam Consultants), as well as all our applicators. Failure to record your spray foam installation can result in license suspensions or removals. UFC provides an online form that can be completed, and we at Polysource also offer a printable version for our applicators to complete on site and then send to Urethane Foam Constants.
DC315 Fireproofing Thermal Barrier Paint also requires a "DC315 Work Completion Record". International Fireproofing Technology Inc. offers an online form that can be submitted, or spray applicators can print a job-site ready form from below. This record should be kept on file with your spray foam business.
